The Australian Type 2 Diabetes Risk Assessment Tool was developed by the Baker IDI Heart and Diabetes Institute on behalf of the Australia State and Territory Governments as part of the COAG initiative to reduce the risk of type 2 diabetes. © 2010 Commonwealth of Australia.

The AUSDRISK screening tool provides an estimate of a person's type 2 diabetes risk. It is not a substitute for medical advice. You may still develop type 2 diabetes even if you have none of these risk factors or demonstrate a low risk score.

It should be noted that AUSDRISK predicts five year risk of diabetes, and the risk of developing diabetes over a longer period would be greater.

A person receiving a high risk result should see their doctor for a clinical test. A high risk result does not mean that an individual has diabetes.

By accepting the website terms and conditions, you are confirming your consent for Diabetes SA to:

  1. store your AUSDRISK score
  2. send your AUSDRISK score to your doctor
  3. send you information about healthy lifestyle
  4. follow up with a call or email if your score is 12 or above

Diabetes SA takes all steps necessary to comply with its legal obligations, including those set out in the Privacy Act 1988 (Cth) (the Act) and the Australian Privacy Principles that are prescribed by the Act. Diabetes SA collects your personal information (which may include health and other sensitive information) directly where possible but may collect information from your carer or guardian.

We collect your personal information for the purposes of our activities, including administering memberships, providing and marketing Diabetes SA and National Diabetes Services Scheme programs and services, processing donations, responding to inquiries and recruitment.

We may disclose your personal information to our service providers, government agencies involved in the provision of our programs or services, where you consent or as permitted or required by law or legal process. Your personal information will only be disclosed overseas with your consent.

If you do not provide us with your personal information, we may not be able to provide you with programs and services or contact you about offerings or opportunities. Diabetes SA’s Privacy Policy (www.diabetessa.com.au/web/privacy) provides more detailed information about the way in which we handle the personal information we collect from time to time, including how to access and seek correction of your personal information or complain about a breach of the Australian Privacy Principles, and how we will handle your complaint.
